For those volunteers interested and scheduled to drive golf carts on the OLPH campus for a speciied event, please complete the form below in advance o your scheduled volunteering date.

If you have not already done so, please see the OLPH office so that we ma obtain a copy of your driver's license.  You may do this on the day of the scheduled event.
